Hospice Spiritual Care
Experienced Hospice Chaplains provide professional spiritual and mental health care for terminally ill patients at home, in a facility or community setting. A unique Spiritual Plan of Care activity program is designed to benefit each patient and their family. Adjustments are made during health decline that continues significant spiritual activity to build inner strength, manage pain and increase feelings of
comfort and peace. Assessments, care plans, charting, calls and reports made as requested by the Hospice provider. Community Chaplains can also follow existing plan of Care and course of intervention previously established by Hospice SC.

Community Chaplains per diem service allows Hospice staff to maintain duties during a patient or family crisis or augment assistance during a period of increased census. If Hospice provider desires, a toll free number is made available to patient and family for spiritual counsel and prayer. This service helps patient, family, facility staff and hospice provider.

Each patient and family are supported in their faith tradition according to individual belief. Catholics are provided Eucharist, Rosary, Sacrament of the Sick with Last Rites; ashes on Lent and other sacrament, Saint and Holy Day traditions.

Provide effective education, leadership and spiritual support to patients and their families during health decline and death. Effective printed items are available for patients and their families for education or upliftment.

Christians receive Holy Communion, living memorial, funeral and special services, singing, regular prayer, Scripture and other activities. Jewish leadership continue tradition, as do Hindu, Buddhist, Muslim and other religions as they support patients and families as desired.

Scripture, music, counseling, special readings, prayer and more help decrease suffering and increase perspective and peace during final days. Family contact, death notification and grief assistance are also available. Inservice programs can be provided on grief, PTSD, disaster, crisis intervention and more.
